Efficacy of whole cranberry powder in the prevention of recurrent UTI

 URINARY tract infections (UTIs) are the most common infection among women and have a high reoccurrence rate. They are caused by bacterial adhesion (bacteria growth) on cells in the bladder. Most infections arise from one type of bacteria, Escherichia coli (E. coli) bacteria. Nearly 1 in 3 women will have had at least one episode of UTI requiring antimicrobial therapy before they reach 24 years of age. One study estimates that nearly half of the women who have had a UTI will experience another one within 12 months.
Such infections are traditionally treated with antibiotics but scientists believe that antibiotic treatments can disrupt the natural occurrence of beneficial bacteria in the body, leading to higher recurrence of UTI. Furthermore, the emergence of antibiotic-resistant strains warrants the need for alternative solutions and preventive strategies. Amongst these alternatives, cranberries are widely documented for their preventive effect against urinary tract infection, thanks to their unique anti-adhesion activity. This activity is primarily due to the presence of key phytochemicals: proanthocyanidins (PACs).
 Whilst PACs are present in other plants, those specific to cranberries have been shown to contain a unique A-type PAC; most other fruits contain only the more common B-type PACs. Researchers have linked the presence of A-type PAC in cranberry to its unique anti-adherence effect against pathogenic bacteria.
Latest in the series supporting the effectiveness of cranberry proanthocyanidins in treating UTI is a clinical study1 published in the peer-reviewed journal Current Bioactive Compounds. Findings by Chatterjee et al. substantiate the efficacy of the daily consumption of 500mg of PAC-standardised whole cranberry powder, PACran, in the prevention of recurrent urinary tract infections.
Developed by Decas Botanical Synergies (DBS), a subsidiary of Decas Cranberry Products, Inc, PACran remains the first and only PAC-standardised whole cranberry powder with clinical documentation.

Supporting urinary tract health
The randomised, double blind, controlled, dose-dependent study examined the effects of PACran supplementation on 60 women volunteers prone with recurring urinary tract infections and a confirmed presence of E. coli in the culture of urine samples as well as mild symptoms of UTI at the time of the study. Two doses of PACran (500 mg & 1000 mg/day) were given the trials during the 90-day treatment period.
First of all, 40% of women from the treatment groups reported complete relief and remission from urological symptoms such as itching and burning sensation during micturition (the impulse to urinate) and frequent urination. No symptomatic relief was reported in the control group.
These observations were supported by urine culture analysis showing between 25 and 45% reduction of subjects with confirmed E. coli load after 10 days of treatment. This effect was persistent over the 90-day treatment period. On the other hand, there was no significant change in the bacterial load of the untreated group at the end of the 90 days.
The authors conclude that PACran is effective in reducing the number of E. coli-positive subjects and in ameliorating the symptoms of UTI in these subjects and that "PACran can be considered as an adjunct to antibiotic prophylactic therapy against recurrent UTI."
The significant clinical results are supported by a trial performed at Rutgers University (New Jersey) on healthy individuals, showing that the consumption of 500mg PACran conferred the same level of ex vivo urinary anti-adhesion activity against uropathogenic E. coli over a 24-hour period as 300ml of cranberry juice cocktail containing 36mg proanthocyanidins.
